We want to make you aware of resources being provided by the Centers for Medicare & Medicaid Services Office of Minority Health (CMS OMH) through their health literacy education initiative Coverage to Care (C2C) that may be useful to your patients.
Through C2C, CMS OMH helps people understand their health coverage and connect them to the primary care and the preventive services that are right for them. C2C includes a variety of resources in as many as 9 languages, with some that may be of interest to your patients, including a resource designed to help patients understand their drug coverage and prescriptions, and a suite of resources to help patients and providers understand chronic care management.
