Provider complaint tool for Medicare Advantage

In case you missed it, the Centers for Medicare & Medicaid Services released a tool for providers to send complaints to CMS regarding Medicare Advantage plans. The form captures basic information about the complainant/provider, beneficiary, and Medicare Advantage plan, a complaint summary, and optional fields for date(s) of service and claim number.
