We want to make you aware of resources being provided by the Centers for Medicare & Medicaid Services Office of Minority Health through their health literacy education initiative, Coverage to Care, that may be useful to your patients. The C2C initiative helps people understand their health coverage and connect with the primary care and preventive services that are right for them.
The C2C initiative offers a diverse range of resources in up to nine languages, some of which may be of interest to your patients. These resources include a guide to understanding drug coverage and prescriptions, and a suite of resources for both patients and providers on chronic care management. Check out the resources here.
