We want to make you aware of resources being provided by the Centers for Medicare & Medicaid Services Office of Minority Health through their health literacy education initiative Coverage to Care that may be useful to your patients. The C2C initiative helps people understand their health coverage and connect with the primary care and preventive services that are right for them.
C2C includes a variety of resources in as many as nine languages, with some that may be of interest to your patients, including a resource designed to help patients understand their drug coverage and prescriptions, and a suite of resources to help patients and providers understand chronic care management.
