Enrollment for ACCESS to Pharmacy Care is now open to community and long-term care pharmacies that are NCPA members and/or participating CPESN® pharmacies! This program is designed to fit into existing pharmacy workflows while creating new revenue opportunities tied to chronic condition care and measurable patient outcomes.
“ACCESS to Pharmacy Care represents a defining opportunity for pharmacy to demonstrate its value in improving patient outcomes at scale,” said NCPA CEO Douglas Hoey in a news release. “This program creates a practical pathway for pharmacies to deliver the kind of ongoing chronic condition support patients need, while building a sustainable model for pharmacy-delivered care.”
