The discussion will be facilitated by LTC Committee member Patrick Chancy, joined by CPESN Pharmacy Care at Home Program Lead Leanne Haley-Brown. Together, they'll take a deeper look into how to successfully follow patients home with long-term care services after their discharge from a hospital or long-term care facility.
