John Croce is a pharmacist and owner of Four Corners Pharmacy in Delmar, N.Y. and a luminary of CPESN of Upstate New York. Croce says he takes pride in the work CPESN is doing. "(I'm proud of) the collaboration of independent pharmacies working toward the goal of improving patient outcomes and reducing health care costs." They're able to do this by having their network be part of a Medicaid project that provides financial support to network pharmacies, allowing them to provide medication management to behavioral health patients, Croce says.
CPESN of Upstate New York is currently working to support CPESN sites as they collect documentation of clinical work through eCare plans that show the value of pharmacy services in both quality of patient care and lowering overall costs. Croce also stated he is "supporting a clinical student rotation for a local pharmacy college, allowing pharmacy candidates to practice at the top of the profession while applying enhanced services to pharmacy patients."
Each week, we feature a CPESN Luminary, highlighting their important, ongoing work in local networks and why being a part of CPESN networks matters.
