Diabetes remains one of the most prevalent chronic diseases, contributing to serious health complications and increased health care utilization when left unmanaged. Independent community pharmacies play a vital role in helping patients manage diabetes through medication optimization, glucose monitoring support, diabetes self-management education and support (DSMES), and regular follow-up.

Through diabetes-focused grant opportunities, NCPA's Innovation Center helps pharmacies expand innovative, patient-centered services that improve disease management and reach individuals at greatest risk for poor outcomes.
