We want to make you aware of patient resources being provided by the Centers for Medicare & Medicaid Services’ Office of Minority Health through their Coverage to Care (C2C) health literacy education initiative. Through C2C, CMS is aiming to help people understand their health coverage and connect them to the primary care and preventive services that are right for them. C2C includes a variety of resources in as many as nine languages, including one designed to help patients understand their drug coverage and prescriptions as well as others dealing with chronic condition management.
