Congress is currently working to pass the Build Back Better Act. NCPA is fighting to ensure they include a provision to finally end DIR fees.
PBMs use DIR fees to extract billions of dollars every year from community pharmacies. The fees don’t improve healthcare for a single patient. They are assessed long after the point of sale, making it impossible for pharmacists to anticipate expenses or manage cash flow. And they go up every year without warning or explanation. DIR fees have increased by 91,000 percent in recent years. Unless Congress ends DIR fees, community pharmacies will be forced out of business and patients will lose access to their preferred healthcare providers. Ending DIR fees will reduce out-of-pocket costs for patients by $9.1 billion over 10 years.
